• Application for Retirement Benefits – This is the standard Application for Retirement benefits which must be filled out completely, along with all accompanying forms, in order to make an application for pension benefits.
• Application for Pension Benefits by an Alternate Payee – Alternate payees should use this form to apply for Pension Benefits. Please read the form carefully as additional documentation may be required.
• Application for Death Benefits – Use this form to apply for death benefits. Please read the form carefully as additional documentation may be required.
• Change of Address Form – Use this form to change your address. Be sure to fill it out completely and return it to the Trust Office.
• Electronic Deposit Authorization (ACH Credits) Form – Use this form to authorize electronic deposits of benefit payments.
• Retiree Voluntary Deduction Authorization – Use this form to authorize deductions from your monthly pension payment to cover the cost to maintain medical, dental, or other benefit programs.
• Spousal Consent Form – Spouses must use this form to consent to the optional pension form selected by an applicant. It must be signed before a notary public.
• California Personal Income Tax Withholding Election Form – It is possible to have California Personal Income Tax withheld from your pension benefit check each month. Use this form to authorize a deduction each month.
• 2024 Form W-4P – Include this form with the Withholding Certificate for Pension or Annuity Payments if you elected to have Income tax withheld from your pension or annuity.

• January 2016 Required Pension Plan Notices and Plan Amendment – Legally required series of government notifications, notices as to the
  Plan’s funding status, and a Plan Amendment.
• December 2015 Notice of Critical Status for the San Diego Electrical Pension Plan – On January 14, 2016, the Board of Trustees elected that the Plan be in Critical Status for the Plan Year beginning October 1, 2015. Federal law requires that you receive this notice.
• January 2011 Notice on Revised Enrollment and Record Cards/Under Age 26 Dependent Re-enrollment
• May 2007 Notice to All Participants and Retirees – Regarding Pension Funding Legislation, QDROs, and Post-Retirement Employment.
